A signed and submitted Policies and Procedure forms must be read and the Annual Registration fee is included in the form. The Authorization Card Form can be downloaded, filled out, scanned and sent to bennettacademy1996@gmail.com. We hold your card just in case website emergencies. Signing the jotform also means you agree to all the contents and documents therein. You can download the documents to hold for a copy for yourself.
